How to handle health care as a nomad?

I’m planning to take a year-long road trip across the US next April visiting friends & working on advancing my mycology career (leading workshops, doing talks, and writing a book). I’ll be converting the back of a minivan to sleep in when I’m not staying with friends or family.

The one thing standing in my way is health care. I have more than 10 active prescriptions, and a few chronic conditions that require regular care.

I’ll be insured in Virginia through December, but it will be pretty hard to find providers (besides emergency rooms) who can see me using that insurance. And I have no idea where or how to buy insurance as someone without a permanent address when my current insurance runs out.

Any van dwellers or other long-term travelers with chronic health issues? How do you get the treatment you need without a permanent address when out of pocket costs for doctor’s appointments and prescriptions medication are hundreds or thousands of dollars?
