⚖️ The Thirlwall Inquiry has found serious failures in safeguarding, management and escalation at the Countess of Chester Hospital’s neonatal unit, concluding that earlier action could have prevented some deaths and injuries.

The inquiry examined events from 2015 to 2018 following Lucy Letby’s trial and convictions. Its remit included parents’ experiences, the hospital’s response to clinicians’ concerns, and wider NHS governance and regulation.

Lady Justice Thirlwall said the report identified a failure to protect babies. It found delays in contacting police, shortcomings in how concerns were handled, and that many parents were kept unaware for years that deliberate harm might have been suspected. 🏥

The final report makes 17 recommendations, including a protocol for suspected deliberate harm and in-cot cameras with remote viewing for parents. The recommendations are aimed at neonatal-unit safety and oversight across the NHS.

The report has now been presented to Parliament. Its findings and recommendations will inform decisions on how hospitals, regulators and government bodies respond to warning signs in neonatal care. 📄
