Middle name on visa
If someone from the USA is applying for the opportunity card do I include my middle name on the first name(s) section or just omit it? As they only ask for the surname and first name(s). Would it be like my passport where it goes
Given names: first name middle name
Surname: last name
Or just first name and last name in their respective spots ?
