A photograph from 1978 depicts Pacific Southwest Airlines Flight 182 just moments before it tragically crashed to the ground following a collision with a private aircraft, resulting in the loss of 144 lives.
It was later concluded that the likely cause of the accident was the failure of the PSA flight crew to adhere to proper air traffic control (ATC) procedures.
The crew of Flight 182 lost sight of the Cessna against ATC instructions to "maintain visual separation from that traffic" and failed to notify ATC when they lost sight of it.
