Post-Tib Pain Three Weeks Out from Trip: Managing Pain vs Managing Expectations

I’m looking for tips on how to manage post-tib pain pre-trip and during the hike beyond what I’ve tried. I have a week to decide to cancel where I can still get some money back. Honest opinions and personal advice are welcome.

Short story: I’m supposed to be headed on my first long walking trip (inn to inn), with about 10-15 miles/day. Most days about 1000 feet of elevation gain. I’m having post-tibialis tendon pain and am considering cancelling. I don’t get a ton of time off and I’m not wealthy. It is unlikely I will be able to afford to do again this decade. This was my dream vacation I finally decided to do and the idea of this trip has literally gotten me through an absolute misery of a year. I feel ashamed, embarrassed, depressed, and like a huge failure.

Long Story: I had been doing really well for a few months with getting in long walks, some hikes with my pack, and working on strength training, but I had a very busy spell at work about a month and a half ago and was essentially sedentary for two weeks (as well as stressed, tense, and poor diet/hydration).

I tried to pick up where I left off and did a couple hour hike. The next day I had a lot of pain around my inner ankle, which led me to learn about posterior tibialis tendon dysfunction. Though this is self-diagnosis, I’m almost positive this is what I have. I have mild pronation on my affected foot, which is larger than my other and had a tendency to develop issues.

I took about two weeks off of working out and focused on yoga and recovery. I’ve since relaxed my training (I used to average 7-8 miles/day, now I’m down to 5), iced, stretched, done stability exercises, got shoes with a slightly higher drop, supportive inserts. I will occasionally have pain free days, but every time I go for a longer hike (8+ miles), it flares up again. Pain is around a 2-3 out of 10 at its worse, but I’m worried about what could happen after a few days on the trail.

Now I’m supposed to leave for this trip in three weeks and I don’t know what to do. It is international, fully paid for, and I can’t get to most of these places with normal transportation.

Are there ways anyone has found useful for managing this type of pain while on a hike? Should I temper my expectations of being able to finish? I know some pain is normal, but I don’t want to injure myself permanently.
