Have a slow leak somewhere and had a few questions about it.

I think the tank is slowly leaking, not sure why yet. Any ideas on what could be leaking there and how to fix it? At first I thought it where the water supply goes into the tank but now I'm thinking its coming from the tank where it sits on the bowl.

The floor is a bit damaged, but doesn't feel squishy and I don't see any mold or anything. Should I be worried about damage to the subfloor?

And finally the floor connection is badly rusted, is this a concern and if so would it be somewhat easy to DIY fix it?
