Have you ever seen this? Sawstop brake…

Over the weekend another employee managed to trigger the brake on the shop saw. Attached is the picture of the blade and brake. In over 20 years, I’ve never seen this happen. 18 teeth sheared off and double stacked in the aluminum caul. What are your theories?
