lockout tagout is one of those things everyone thinks they do right until they dont

Saw a near miss on a job site last month that still bugs me. Electrician was swapping a breaker in a panel, thought he had isolated everything but missed a backfeed from an emergency circuit. Didnt get hit but only because he happened to test before touching - if he hadnt had his meter out it would of been a different story.

The thing that gets me is everyone on that crew had their LOTO training up to date. The issue wasnt knowledge it was rushing. Panel was supposed to be dead, guy trusted the one lock he put on without verifying every source.

I know most of us have been working around live panels longer than we care to admit but its always the experienced guys who get complacent. The apprentices are actually more careful because theyre still scared of it.

Anyone else notice the near misses tend to happen to the senior guys more than the green ones? Always seemed backwards to me.
